The staff of our Patient Navigator Service is specially trained to help you find a new doctor. They will also be happy to schedule your first appointment!

Registration Forms

If you are a new patient, we encourage you to print our registration form and complete it prior to your first appointment date click here. Completing this form before you arrive at the office will speed-up your registration. It will also help to ensure that we have accurate information. If you are an established patient but have had a change to your address, phone number or insurance plan, please bring this revised form with you to your next appointment.

Your medical history is an important part of your medical record with NCH Physician Group. Insurances Accepted
NCH Physician Group accepts most commercial insurances, Medicare, most Medicare Private Fee for Service plans and self paying patients. We do not accept any HMO plans.

Financial Policies 
Depending on the insurance plan, we will collect co-payments, deductibles and charges for non-covered services at the time of service. We will then file an insurance claim to your insurance company. If you are insured by a plan with which we do not participate, we will require payment in full at the time of service and we will then file a claim with your insurance company as a courtesy. We accept cash, check, debit card, Visa, MasterCard, Discover or American Express as methods of payment.

Hours of Operation
Our medical and specialty offices are open from 8:00 AM to 5:00 PM, Monday through Friday and we are closed on holidays. Occasionally, our offices may schedule you for a Saturday appointment if necessary.

If you need to see your NCH Physician Group physician during regular office hours, please contact your physician’s office directly.  During other hours, the NCH Physician Group has an Urgent Care Center located at 801 Vanderbilt Beach Road.

Refilling a Prescription 
Many of our offices have designed their individual phone tree options to direct you to the nurse’s line so that you can expedite a prescription refill. Please contact your physician’s office directly to obtain information on refilling a prescription. Click here for a list of our physicians' office phone numbers.
Scheduling an Appointment
Please contact your NCH Physician Group physician’s office directly to schedule an appointment. Not sure if you need to see a doctor? Your physician’s office can help you. Please do not hesitate to contact them during regular business hours. In some cases, you may be offered a same-day appointment. During hours when our offices are closed, you may wish to visit one of our Urgent Care facilities. Click here for information on our Urgent Care facilities.

If your NCH Physician Group physician orders a medical test, it will in most cases be scheduled by your doctor’s office. However, appointments for laboratory blood or urine tests are not needed. Your physician will provide specific instructions regarding how to obtain the tests that she/he orders.

Do you need to cancel your appointment? We ask that you call the office at least 24 hours in advance of your scheduled appointment time so that another patient can be accommodated.

Contacting Your Doctor 
If you already have an NCH Physician Group provider, and you need to schedule an appointment, renew a prescription or simply ask a question, you can reach your doctor’s office by calling their direct phone line. For a complete list of our physicians' office phone numbers and locations click here, or use the “Our Doctors” tab on the top tool bar. If your phone call is being made during lunch or after hours, you may be routed to our answering service.

In Case of Emergency... 
There are specific conditions that should not wait for a call back from the doctor before calling emergency services. These medical conditions are time sensitive and waiting too long could have serious consequences, even death.

If you are experiencing a possible life threatening symptom (chest pain, shortness of breath, loss of consciousness, severe bleeding) call 911 immediately and describe your symptoms to the operator. Stay calm; be sure to clearly give your complete address and your emergency. Stay on the line until told to disconnect.

Poison Control Center

In the case of a possible poisoning, call the U.S. Poison Control Center number 800-222-1222. An operator will advise you to either administer an antidote or, if needed, they will call an ambulance for you. 

Questions About Your Bill 
The staff in our Central Billing Office is available to answer questions about your bill from 8:00 am to 6:00 pm Monday through Friday. Simply call 239-624-0400 to reach one of our knowledgeable billing specialists if you have questions about your balance, payments or payment plans.
