Does my spouse need to do the same screenings as me? In 2013, your spouse ( if an odd birth year employee), will need to complete the same steps as you during the period October 1, 2012 through September 30, 2013 in order to qualify for 2014. I have an even birth year. When do my spouse and I have to complete Road to Wellness requirements? You and your spouse must complete the RTW requirements during the period October 1, 2013 through September 30, 2014 for January 1, 2015.
If I complete the RTW but my spouse fails to complete the process, will I still be able to participate in the Healthy Choice or the Healthy Partner plan? No. Unless you choose not to enroll your spouse, you will only be able to enroll in the Basic Plan.
If I and/or my spouse do not complete our RTW by the deadline, can we enroll in the Health Choice or Healthy Partner plan when we complete it? No. All requirements for the RTW must be completed by the deadline. You have a year (October 1-September 30) to complete the process. We encourage you and your spouse to begin the process as early as possible. Labs/Biometrics and Health Risk Assessments must be completed by September 1, 2013. Age/Gender screenings, Diabetic Management program and Health Coach Meetings can be completed through September 30, 2013.
Will test results affect my coverage? No. Results are confidential and kept by Community Health Partners who handles our Health Plan Case Management. Findings can not be used to decrease health coverage. Will I still be able to participate in the Healthy Choice or the Healthy Partner plan if my lab results reveal a health problem? Yes, requirements for either health plan are not determined by your health status, rather by participation in the Health Risk Assessment screens, following your Health Coach's recommendations and having your age/gender based screenings with a physician
Can I use my own physician? Yes, for Age/Gender Based Screenings, other conditions needing follow-up per your Health Coach, your Annual Physical, and any medical issues you have. Lab draws for your Annual Physical and your Health Risk Assessment must be done by Occupational Health to insure consistency in data. If you currently do not have a physician, contact Community Health Partners at 659-7760 for the name of physicians in your area, or ask your Health Coach at 659-7703.
I don't remember my user name and password to log on to take my HRA, what is it? Your login ID on the HRA website is your employee ID number. For your spouse, add the letter "S" onto the end of your employee ID number. The password for both employees and spouses is NCH1. (NCH must be entered in all capitals to allow entrance to the site.)
Can I view my own labs on the NCH portal? Yes. Contact the Admitting Office at either campus to sign up for the portal. You will be required to provide your personal email address and a 4 digit pin number that will be used to set up the access. Once the Admitting personnel submits the request for access, an email will be sent to your personal email address (please check spam/junk mail folders.) The email will contain instructions for completing the portal access. Once the portal account is set up it can be accessed by going to www.mynch.org. You will have access to your health information 24 hours a day. If you need assistance with your profile, call 1-877-621-8014 for assistance.
